Job Description

The Medical Records Clerk is responsible for organizing, maintaining, and securing all patient health records in accordance with healthcare regulations and internal policies. This role ensures that medical documents are accurately filed, updated, and accessible to authorized personnel when needed. The position requires a high level of confidentiality, attention to detail, and the ability to work efficiently within a fast-paced medical environment.


Responsibilities:

  • Organize and file patient records both digitally and physically to ensure accurate storage and easy retrieval.

  • Process requests for medical records in compliance with hospital guidelines and data protection regulations.

  • Ensure all medical documentation is up-to-date, correctly coded, and appropriately indexed.

  • Assist in retrieving files for clinical staff and administrative departments upon request.

  • Monitor and manage the flow of patient information while maintaining strict confidentiality.

  • Conduct periodic audits of medical files to identify and correct errors or missing information.

  • Collaborate with healthcare staff to resolve documentation discrepancies or missing data.

  • Maintain logs and generate reports related to patient records and file management.

  • Follow procedures for records retention and secure disposal of outdated or irrelevant documentation.


Requirements:

  • High school diploma or equivalent; additional certification in Health Information Management is an advantage.

  • Previous experience in a healthcare facility or clerical setting is preferred.

  • Knowledge of medical terminology and familiarity with electronic health record (EHR) systems.

  • Strong organizational skills and the ability to handle sensitive data with discretion.

  • Proficient in computer applications including MS Office and medical data software.

  • Ability to multitask, prioritize duties, and maintain accuracy under pressure.

  • Strong communication skills and a customer-oriented mindset.
