Home Care Nurse

We are seeking a highly skilled and experienced Home Care Nurse to join our team. The Home Care Nurse will be responsible for providing direct patient care and support services to individuals in the comfort of their own homes. The ideal candidate will be compassionate, organized, and possess strong clinical skills.

Key Responsibilities:

- Conduct initial and ongoing assessments of patient's needs and create personalized care plans

- Implement and monitor the care plan, making adjustments as needed

- Administer and monitor medications as prescribed by the physician

- Provide wound care and other necessary nursing treatments

- Collaborate with interdisciplinary team members to provide comprehensive care

- Educate patients and their families on disease management, medication administration, and other health-related topics

- Monitor patient's progress and document all relevant information

- Communicate and collaborate with physicians and other healthcare professionals to ensure quality care

- Available for on-call duty and able to handle emergency situations in a timely and efficient manner

- Maintain accurate and up-to-date patient records

- Ensure patient comfort and safety at all times

- Adhere to all state and federal regulations and guidelines

Qualifications:

- Current and valid RN 

- Minimum of 2 years of experience in home care or a related healthcare field

- Knowledge of home health nursing procedures and standards of care

- Strong clinical skills with the ability to think critically and make sound decisions

- Excellent communication and interpersonal skills

- Ability to work autonomously and as part of a team

- Empathy towards patients and their families

- Current CPR and First Aid certification

- Reliable transportation and valid driver's license

- Flexibility to work a rotating schedule, including weekends and holidays
